The enthusiasm is justifiable: AI can clarify medical terminology, propose potential diagnoses and treatment choices, and enable patients to take a more active role in their care, becoming “activated,” which means engaging confidently in managing their treatment and bearing more personal responsibility for adhering to the plans set by their healthcare providers.

Like any disruptive technology, it presents advantages and disadvantages—both opportunities and risks. When used appropriately, AI can serve as a useful companion in your healthcare journey. Conversely, if misused or applied unwisely, it may lead to misguidance or potentially harmful outcomes. The essential factor is achieving a balance: leveraging the benefits while being mindful of the inherent risks.

Nonetheless, AI lacks genuine feelings. It does not experience love, fear, or care. It mimics compassion by analyzing language trends. When a chatbot reassures a patient with, “You are not alone in this,” it is not conveying genuine concern but operating from agentic AI. Communicating from agentic AI is akin to a blind person describing color, trained on empathetic language but lacking firsthand experience of connection or community.

The conversation is ongoing. Some contend that simulated empathy is insincere and could mislead patients, creating a false sense of real presence, particularly for those who are vulnerable. Others argue that the subjective experience is what truly matters, and if AI brings comfort, there are no significant objections to its application. Rather than framing AI as speculative insight, one might regard it as a communicator, sharing collective empathy embedded in text. It articulates beautiful sentiments shaped by multitudes, not by individual emotion. Careful and informed application is crucial. Emerging studies indicate that AI tools sometimes exceed humans in perceived empathy. Experimental chatbots like Woebot and Therabot hold promise for mental health assistance, enhancing outcomes for those grappling with anxiety and depression.

This discussion reflects a persistent tension in clinical practice: the role of emotional detachment. Medical professionals are often trained to maintain emotional boundaries—not out of a lack of compassion, but because excessive emotional involvement can lead to burnout or impaired judgment. They articulate the correct phrases, adopt a compassionate tone, and offer solace, sometimes driven more by obligation than emotional sincerity. This aligns with the Stoic notion of apatheia, achieving calm clarity in the service of others. However, excessive detachment can morph into emotional labor or moral injury when clinicians feel compelled to feign care without sufficient time or support to genuinely feel it.

In this framework, AI’s enactment of empathy may reflect not only the form of medical compassion but also its most profound ethical dilemmas. So, if we are willing to accept this form of professional empathy from human providers, should we also be open to receiving it from machines, particularly if it could improve results?

The Hippocratic Oath encourages physicians to do no harm, yet the effects of empathetic AI on mental health are still uncertain. Patients might develop emotional reliance, overlook medical advice, or refuse critical treatment. Granting AI autonomy risks undermining clinical discretion. If patients find it easier to confide in bots than in healthcare providers, or if clinicians delegate challenging discussions to AI, we jeopardize the trusted, fiduciary foundation of medicine. Doctors must maintain authority to ensure ethical, safe, and patient-centered care.
